The post holder will beTobe responsible for the delivery of nursing services, working as part of thepractice multidisciplinary team, delivering care within their scope of practiceto the entitled patient population. The practice nurse willbe responsible for a number of clinical areas such as health promotion, chronicdisease management, health prevention, well women and well man clinics, as wellas supporting the management team in the reviewing of clinical policy andprocedure.
You will join a clinical team consisting of 2 Partners, 4 salaried GPs, 4 Practice Nurses, 2 Associate Nurses, 4 HCAs, and 1 Clinical Pharmacist.

Job Title: Practice Nurse/Specialist Nurse/Community Matron
Brief Description:
Responsible for ensuring the delivery of safe andeffective nursing care including treatment, preventative care, screening, assessments, home visits andpatient education to the whole practice population. The post holder isaccountable for nursing service delivery and will work closely with the Lead Nurse and GPs to deliver the practice priorities.
Main Duties and Responsibilities:
Responsible for meeting the nursing needs andproviding nursing care to patients, including the provision of general andspecific health assessment and screening to the practice populationindependently and in consultation with the general practitioners.
To demonstrate the rationale and evidencebehind various methods chosen to meet health care needs.
To be involved with practice policy developmentand to provide nursing input to decisions affecting the care ofpatients. Keeping up to date clinical managementprotocols, continuing audit cycles and liaising with the Lead Nurse and Practice Manager regarding deadline achievements.
To participate in the administrative andprofessional responsibilities of the practice team.
To review and implement infection controlmeasures and updating clinicians at clinical meetings.
To participate in education, training, teachingand support of students and staff of all disciplines within the practice.
To maintain professional development byattending courses and study days as agreed with the employer.
To maintain accurate patient records andstatistics on the computer system and in the patients notes. Developingon-going IT skills to compliment good standards for dissemination to allclinicians.
Control and maintain medical supplies andequipment used to agreed standards, including the sterilisation of instruments.Keeping inventories for all clinical areas to facilitate stock controlmethodology. Calibration and care of equipment whilst maintaining records ofmaintenance, shelf life and replacement need.
To advise patients on general health care andminor ailments, referring to the general practitioners as necessary.
To provide a woman's health service, to includecervical screening and advice and information regarding breast screening;Chlamydia testing, contraceptive pill checks, administration of Depo Proveracontraception injections and sign-post for contraception consultation/changes.
To assess, diagnose and prescribe appropriatelywithin your capability and nursing grade.
To care and educate patients living withchronic disease, in particular diabetes and hypertension inline with NICE guidelines and the quality outcomes framework. This includeshome visits for housebound patients with chronic disease for their annualreviews and annual immunisations.
Assessment and care of wounds, removals ofsutures/clips, administration of injections, venepuncture, Receiving andinterpreting microbiology results, following up those who need treatment.
Compression bandaging and hosiery Doppler andMonofilament assessments Wound Care plans including the completion of woundassessment documentation/templates and referrals to TVN
Taking of cervical smears, maintaining records,issuing results and following up those patients who need referral.Contributing to the cytology recall system for the health authority and thepractice.
To lead on the delivery of the nationalchildhood immunisations programmeaccording to current schedules and any catch up programme The accuraterecording of the information in the child health records and to participate in measures (such as education sessions) toimprove the uptake of childhood immunisations, especially the Measles Mumpsand Rubella vaccine.
To deal with telephone enquiries/EMIS tasks andprovide advice for nursing related queries. This is to include the booking ofappointments and ensuring any additional paperwork such as preparation ofblood or specimen forms for investigation by the pathology laboratory arecompleted prior to appointments etc.
Prioritise health problems and intervene appropriately to assist thepatient in complex, urgent or emergency situations, including initiation ofeffective emergency care
EMIS alerts completed when in consultation withpatients.
To chaperone patients where necessary
The undertaking of basic observations, smokingdata/smoking cessation advice/signposting, alcohol intake, body mass index,blood glucose monitoring, ECGs, Frailty scoring.
Home visits and assessments.
Ear assessments, hearing checks and earirrigation if clinically indicated.
NHS Health Checks Health Promotion DiseasePrevention Management/Intervention Review HbA1c and Cholesterol bloodresults
EMIS templates to be completed holisticallywith individualised information free texted into the consultation and correctREAD codes, plans of care completed and saved to EMIS with review/follow updetails and shared with the patient/carers for continuity of care.
Any other duties appropriate to the role
