About the job

We are seeking an enthusiastic, innovative, and adaptable Care Coordinator to join our team. This role is central to ensuring clients receive the right support from the right workers while maintaining excellent service standards.

Responsibilities
  • Care Coordinator – Manage client care/support requests from start to finish, ensuring workers are allocated based on skills, availability, and client preferences.
  • Staff Matching – Proactively match workers to clients/packages to ensure the best fit and continuity of care.
  • Client & Worker Engagement – Build and maintain positive relationships with clients, their families, and workers.
  • Scheduling & Rota Management – Organise shifts and placements to ensure all packages are fully covered.
  • Quality & Compliance – Ensure placements comply with CQC regulations, safeguarding policies, and internal standards.
  • Problem Solving – Act quickly to resolve last-minute staffing changes or placement issues.
  • Project Participation – Collaborate with colleagues and management on new initiatives, service improvements, and business development projects.
  • Target Achievement – Work towards agreed monthly and quarterly goals, with performance incentives in place.
  • Process Improvement – Identify and implement ways to enhance efficiency, quality, and the client/worker experience.
  • Record Management – Keep accurate and up-to-date records in our systems.
