Job description
Overview

We are looking for forward thinking, dynamic team players who are HCPC registered. You should have the ability to work within an MDT and manage an independent caseload of sometimes complex patients. To assess, diagnose, plan and deliver treatment programs to patients in your care. Our lively multi-disciplinary unit is committed to providing excellent patient-focused care. Within each rotation you will be supported closely by a multi-disciplinary team, allowing you to develop clinical knowledge and skills. We are committed to CPD and provide tailored IST programmes, an active peer support group and regular appraisals. The post will include on calls once competent and weekend working.

The post holder is expected to rotate between Central Intermediate Care Bedbase and Homepathway. Work pattern is Early 7:30am to 15:30pm; Late shift 10:00 - 18:00.

Responsibilities
  • Competently assess patients, identify problems and agree objectives of treatment with the patient.
  • Clinically reason, plan and carry out appropriate treatment programmes and techniques using evidence-based practice, and identify safe and timely discharge plans.
  • Plan and prioritise own workload according to clinical need, and discuss relevant issues with a senior member of staff.
  • Participate in late call and weekend working as scheduled.
  • Understand roles and work with the wider MDT.
  • Maintain confidentiality, dignity and professionalism at all times.
  • Maintain a high standard of documentation.
  • Work well as part of a therapy team and the wider MDT.
