Overview

We are looking for an enthusiastic and creative Physiotherapist to work in our Buckinghamshire Community Learning Disability Health Team. This is a full or part‑time position for HCPC registered Physiotherapists with an interest in community LD services. The successful candidate will work as an integral member of a diverse multi‑disciplinary team and will join the growing Physiotherapy service. This is a community role for a band 5 Physiotherapist and involves visiting service users at day centres, clinic spaces and their homes. A passion for working with adults with learning disabilities in this varied and interesting role is essential. The team has office bases in Aylesbury and High Wycombe. The role is county wide and we can adapt work locations, workdays and hours to support your circumstances.

Key Relationships
  • Work with other team members of both the Physiotherapy team and wider MDT.
  • Work with service users and carers within a variety of community‑based settings across Buckinghamshire.
  • Liaise with voluntary or other organisations as appropriate.
Duties and Responsibilities
  • Manage a caseload, using evidence based/client principles to assess, plan, implement and evaluate interventions in a variety of community‑based settings.
  • Maintain clinical records.
  • Develop skills and knowledge through participation in the Band 5 Physiotherapy Development Programme and competency framework.
  • Gain experience through involvement in CPD activities and on‑job learning opportunities.
  • Plan and implement individual and/or group interventions, in collaboration with the service user, using appropriate interventions to achieve therapeutic goals.
  • Undertake Physiotherapy assessments for a designated caseload, addressing all areas of complex physical health as appropriate with support where necessary.
  • Work with service users to identify Physiotherapy goals as part of the overall care plan.
  • Use appropriate outcome measures to effectively evaluate physical outcomes for the service user and the Physiotherapy service.
  • Assess service users' understanding of interventions, gain valid consent and work within a legal framework with service users who lack capacity to consent to treatment.
  • Maintain accurate and timely record keeping in line with legal, professional and Trust requirements.
  • Carry out assessments and manual treatments of service users with moderate physical effort and comply with the Trusts Manual Handling Policy at all times.
Qualifications and Experience
  • Physiotherapy Degree
  • HCPC registration
  • Desirable: Membership of CSP
  • Desirable: Experience of working with people with mental health problems/learning disability.
  • Desirable: Completed a range of student placements providing some experience of community based working.
  • Desirable: Desire to learn and expand experience within learning disabilities and complex health.
  • Desirable: Some experience and understanding of MDT/team working.
  • Desirable: Experience with some professional assessment tools.
  • Desirable: Documented evidence of continuing professional development.
  • Desirable: Previous placement in learning disabilities.
Physical Skills and Additional Requirements
  • Full driving licence with access to car unless you have a disability as defined by the Disability Discrimination Act 1995.
  • Computer literacy including basic proficiency in using Word, Excel, e‑learning packages, Electronic Patient records and Powerpoint.
  • Working alongside service users, often with complex manual handling, to support them in undertaking activities and treatments identified in their care plan.
  • Desirable: Clean Driving Licence, Strong written and verbal communication skills.
  • Desirable: Ability to work in a team.
  • Desirable: Some knowledge of current best practice in Physiotherapy.
  • Desirable: Ability to manage own workload and determine priorities.
  • Desirable: Ability to respond and adjust to service users' needs.
  • Desirable: Basic knowledge of augmentative and alternative communication (AAC) and communication difficulties.

We are a supportive AHP team that work across the Trust with LD Physiotherapy colleagues in Buckinghamshire, Hertfordshire and Norfolk and you would have many CPD opportunities and colleagues to work with. As a Physiotherapist in the Mid West Essex Physiotherapy Team you would be offered a structured induction programme, given the confidence and skills to carry out the role, part of a Physiotherapy team made up of Physiotherapy Team Leads (B7) and Physiotherapy Support Workers (B4), receive regular clinical supervision and be encouraged to participate in CPD. You would be part of an MDT that values Physiotherapy and part of a wider Physiotherapy network across our Mental Health and LD services. You would work autonomously and use your creative skills, and be an important part of an innovative, creative and developing service.
