Job description

Boyatt Wood Surgery and Eastleigh Medical Practice are looking for a keen and committed Pharmacy Technician, with excellent communication skills and knowledge to join our multidisciplinary general practice team.

We are looking to recruit a Clinical Pharmacy Technician (Band 5 equivalent pro rata with NHS Pension available) to enhance the care of our patients. This role provides a fantastic opportunity for career progression and development.

The post will complement our existing team of two Pharmacists and our multidisciplinary team of 8 ANP/ACP , 8 GP and 2 Nurses who will provide professional support and development.

We will welcome applications from all sectors of Pharmacy, and you will be fully supported with a comprehensive induction and enrolment in the Primary care pharmacy education pathway provided by CPPE if not already completed.

Main duties of the job

To provide specialist pharmaceutical services, professional advice and direct support to Practice GPs, Nurses, Pharmacists, Pharmacy Technicians, HCAs and Reception/Administrative Staff and the wider multi-disciplinary team to promote the safe, effective, and high-quality, cost-effective use of medicines

To undertake medicines reconciliation in accordance with local protocols. To update patients' medical records and prescribing accordingly, referring on any discrepancies to the clinical pharmacist, ANP/ACP or GP.

To liaise with patients, clinical and non-clinical staff, practice, hospital and community pharmacists/ technicians and patient's carers to improve the pharmaceutical care of patients.

To support the clinical pharmacists to ensure the delivery of safe, effective systems for repeat prescribing, medicines optimisation, reducing medicines waste and maximising patient outcomes.

To support the clinical pharmacists in providing structured medication review to those on multiple medications/polypharmacy and in care home supporting the practice with implementing the PCN Directed Enhanced Service (DES).

Boyatt Wood Surgery is a partner practice about a mile from central Eastleigh. Eastleigh Medical Practice currently has over 14750 registered patients and Boyatt Wood has 8500 patients. Job responsibilities

MAJOR DUTIES AND RESPONSIBILITIES

To undertake medicines reconciliation in accordance with practice protocols and within clinical competence to update patients' medical records accordingly referring on any discrepancies promptly to the clinical pharmacists, ANP/ACP or GPs

To provide advice to our practice team (e.g. GPs, Nurses, HCAs, Practice Management, and other non-clinical staff) concerning changes to prescribing to support the improvement of prescribing safety, quality, and cost-effectiveness.

To work with reception staff and help manage repeat prescription requests and resolving medication queries.

To liaise with community pharmacy to organise supply of items that may be difficult to obtain.

To work in conjunction with the nominated clinicians and clinical pharmacists to support medicines optimisation in nursing homes.

To support the clinical pharmacists to provide structured medication reviews, patient information, medicines awareness and to undertake domiciliary visits if required

To develop recall systems to ensure that all patients have appropriate therapeutic drug monitoring.

To respond to medication queries from patients providing them with advice and support to best optimise and manage their medication, referring on patients who need further management from a clinician or clinical pharmacist.

To participate in medication audit and use medication safety tools such as PINCER and Polypharmacy Risk Identifier Tool (PRIT) to identify patients at risk of adverse outcomes.

To address the following areas with support from the clinical pharmacy and PCN prescribing team to promote the safe supply of medication:

Review dates following medication reviews.

Conduct medicines reconciliations in appropriate patients

Facilitate and document correct addition & removal of medications following receipt of a discharge summaries, secondary care, and community clinic letters.

Tidy repeat medication lists including synchronising durations and removing medications not ordered within a defined period.

Co-ordinate District Nurse Medication and Dressing requests for the practice population

Co-ordinate medical device prescription requests, including requests for catheters, nutritional supplements/feeds, and stoma care products.

To assist in the appropriate monitoring and management of prescribing budgets and to liaise with the ICB medicines optimisation team.

To analyse practice prescription data to produce reports, as requested.

To review practice stock management and develop systems to improve processes.

To participate in review and setting of policies relating to medicines management and prescribing

To implement initiatives to reduce waste of prescribed medicines and participate in health promotion campaigns.

To support the practice clinical, management and administrative teams to implement procedural and service changes relating to prescribing to ensure best practice is adopted in accordance with local guidelines, legislation, directives, and safety updates e.g. NICE, MRHA etc.

To organise and support various systems for the delivery of QOF (Quality and Outcomes Framework) and local commissioned advanced services.

To attend and participate in local meetings when required.

Experience
  • Medicines optimisation or dispensing role in a community, primary care or a hospital setting.
  • Use of general Practice repeat prescribing system EMIS Web
  • Skill in analysing and presenting prescribing activity data
  • Experience of patient facing medication counselling / role
Qualifications
  • NVQ Level 3 in Pharmacy Services or equivalent.
  • Registration as a Pharmacy Technician with GPhC
  • CPPE PCPEP or equivalent.
  • Qualified in Phlebotomy
