Salary: £115 Per Day | Hours: 37 Hours | Working Schedule: Monday to Friday | Contract Length: 3 Months

Responsibilities
- Provide a Personal Advisor service to Category 1 (eligible), Category 2 (relevant) and Category 3 (former relevant) young people to help them move to independent living, including practical advice.
- Offer advice and support to Category 4 (reconnecting to care), Category 5 (previously subject of a SGO) and Category 6 (previously Looked After but not qualifying as a care leaver) young people where appropriate.
- Co-work with Category 1 and 2 young people and case manage Category 3 (4, 5, 6) young people.
- Be supervised by the 16 Plus Senior Practitioner and managed by the Team Manager within the 16 Plus Service; provide an independent service.
- Provide advice, guidance and practical support to promote independence, including budgeting and assisting with purchasing items for own property.
- Complete the My Pathway Planner with young people and assist social workers in the assessment, preparation and reviews of the Pathway Plan.
- Attend pathway plan reviews and promote the wishes and feelings of young people; coordinate services identified in the Pathway Plan.
- Maintain regular contact with young people, become a key worker for those ceasing to be looked after, and support young people who may be known to Youth Offending Service or Probation.
- Provide written and verbal reports to assist in assessing and monitoring progress; develop effective working relationships with internal and external colleagues.
- Ensure needs are met, inform the case manager if specific needs are not being met, and respond flexibly to crisis situations.
- Record information accurately and timely; inform the Team Manager if needs or circumstances become too complex for a personal advisor to manage.
- Work with other professionals and outside organisations; participate in supervision, training and professional development; be available on a rota basis for office duties and flexible hours.
Administration
- Arrange access to translation, interpreter and advocacy services as appropriate.
- Use information technology to maintain current evidence planning, consultation, actions taken and monitoring of outcomes.
- Maintain accurate case records in the Social Services database and comply with relevant procedures and policies.
- Contribute to team effectiveness, meet targets, participate in duty rotas and take part in supervision as appropriate.
