We are looking to recruit a highly motivated and enthusiastic Clinical Pharmacist to work alongside and provide complementary skills to our existing clinical teams whilst building strong effective working relationships with local community and ICB Medicines Optimisation Team to improve care and services to our patients.
The successful applicants will have good clinical backgrounds and possess the requisite skills required to support safe, evidence based, clinically and cost-effective prescribing and provide support to general practice clinicians.
Experience in general practice is desirable but not essential and qualification as an independent prescriber would be an advantage. As a PCN pharmacist you will receive fully accredited training and will be provided with appropriate supervision and guidance.

Hatters Health PCN, a collaborative of 5 friendly, pro-active practices in Luton, have an exciting opportunity for a hardworking, enthusiastic and innovative clinical pharmacist to join our expanding Pharmacy team. The role will involve working independently and within a highly motivated additional roles team to support delivery of PCN objectives including structured medication reviews and to ensure enhanced outcomes for our patients. The successful candidate will be an excellent communicator, highly organised, professional who is looking to join a dynamic team working to support a diverse range of people from different cultural and social backgrounds. You will benefit from regular tutorials and clinical supervision including support to undergo CPPE pathway in primary care will be provided as required. The role provides the opportunity to be actively involved in how your role evolves over time including development in a specific long-term condition and the opportunity to work within a highly motivated, dedicated multi-disciplinary team. Manage patients within a specialised scope of practice i.e. a long-term condition of interest includes frailty. Carrying out structured medication reviews and optimisation of the condition. Undertake clinical medication reviews with patients and produce recommendations for review. Undertake clinical medication reviews with patients and produce recommendations for the senior clinical pharmacist, nurses or GPs on prescribing and monitoring. Work with care home staff to improve safety of medicines ordering and administration. Answers relevant medicine-related enquiries from GPs, other practice staff, other healthcare teams and patients with queries about medicines. Suggesting and recommending solutions based on guidance. To reconcile medicines following discharge from hospitals, intermediate care and into care homes, including identifying and rectifying unexplained changes. Ensure that patients are referred to the appropriate healthcare professional for further management where outside scope of practice Identification of cohorts of patients at high risk of harm from medicines through pre-prepared practice computer searches. This might include risks that are patient related, medicine related, or both. Contribute pharmaceutical advice for the development and implementation of new services that have medicinal components Analyse, interpret and present medicines data to highlight issues and risks to support decision making. Undertake clinical audits of prescribing laid out in the Primary Care Incentive scheme PCN DES and in relation to any other projects initiated by the PCN. Implement changes to medicines that result from MHRA alerts, product withdrawal and other local and national guidance. Monitor practice prescribing against the local health economys RAG list and make recommendations to GPs for medicines that should be prescribed by hospital doctors (red drugs) or subject to shared care (amber drugs). Auditing practices compliance against NICE technology assessment guidance. Provide newsletters or bulletins on important prescribing messages. Provide education and training to primary healthcare team on medicines optimisation and safer prescribing. Work with the general practice team to ensure the practice is compliant with CQC standards where medicines are involved. To support public health campaigns. To provide specialist knowledge on all public health programmes available to the general public.
