Working within the Primary Care Network Team, the postholder will support the Multi-Disciplinary Team in delivering high-quality patient care. This role involves supporting assessments, implementing treatment plans, and contributing to the efficient running of the service.

Main duties of the job
  • Work effectively as part of the Multi-Disciplinary Team within the Primary Care Network and wider GP network
  • Apply risk assessment and management principles to all clinical work
  • Exercise initiative and independence in carrying out delegated activities at a Professional level
  • Provide clinical and administrative support to the MDT as required
  • Effectively triage referrals and signpost to relevant professions within the Primary Care Network

Nexus MK PCN offices are based in Linford Wood Milton Keynes and cover 5 practices which include Oakridge Park Medical Centre, Wolverton Health Centre, The Stonedean Practice, Sovereign Health Centre, Purbeck Medical Centre.
