Working within the Primary Care Network Team, the postholder will support the Multi-Disciplinary Team in delivering high-quality patient care. This role involves supporting assessments, implementing treatment plans, and contributing to the efficient running of the service.
Main duties of the job
- Work effectively as part of the Multi-Disciplinary Team within the Primary Care Network and wider GP network
- Apply risk assessment and management principles to all clinical work
- Exercise initiative and independence in carrying out delegated activities at a Professional level
- Provide clinical and administrative support to the MDT as required
- Effectively triage referrals and signpost to relevant professions within the Primary Care Network

Essential Knowledge and Skills
- Excellent organisational and time management skills with ability to prioritise tasks
- Comprehensive understanding of the Occupational Therapy role
- Ability to follow local lone working policies and procedures
- Experience working in healthcare settings
- Experience supporting needs associated with activities of daily living
- Proven ability to work independently
- Experience working within multi-disciplinary teams
- Previous experience supporting Occupational Therapists/Physiotherapists
Essential Qualifications and Requirements
- NVQ Level 3 in Health or equivalent qualification
- Proficiency in Microsoft Office applications
- Evidence of continued professional development
- Access to own vehicle with business insurance for community/home visits
- Demonstrated computer literacy
- Knowledge of therapeutic interventions for adults with frailty, long-term and complex medical conditions
- Awareness of relevant healthcare legislation
- Experience in delivering therapy groups
- Community healthcare experience
