Part-time Hybrid/Onsite Primary Care Pharmacy Technician, Durham

We currently have an exciting opportunity for a Hybrid/Onsite based Pharmacy Technician due to company growth and expansion to work part-time (18.75hrs) for a practice in Durham . This opportunity is available for highly motivated and focused Pharmacy Technicians, who will be responsible for the smooth running of day-to-day activities within general practice splitting their time between a few days onsite and few days working from home, whilst playing a vital role in providing clinical support and advice to patients focusing on best practice and adhering to safety guidelines.

The work pattern split of onsite and remote working can be negotiated.

Part-time hours can be worked within core opening hours of the GP practice.

Main duties of the job

As a hybrid/onsite Pharmacy Technician you will confidently act within professional standards and boundaries, supporting and working alongside your General Practice team as well as Pharmacists and Pharmacy Technicians within TPN.

You will help resolve medication-related queries, work as part of a multidisciplinary team, improve patient outcomes and support the improvementof access to healthcare.

Job responsibilities
  • Medication Reconciliations using clinical letters and post hospital discharge summaries
  • Medicine Management including Prescription screening and processing
  • Respond and communicate to patient related medication queries
  • Communicate with local pharmacies regarding stock supplies issues and shortages
  • Medication synchronisation in line with regular repeat prescription
  • Medication monitoring recalls (e.g BP, blood tests, annual reviews)
  • Prescribing audits in line with CQC/QOF targets as set by the Prescribing Team (training can be provided)
  • Standard/Annual medication monitoring
  • Shared Care Protocols
  • Action medicines safety alerts from CAS/MHRA/ICB
  • Level 1 and 2 Medication reviews (training can be given)
  • Assist with Pre-SMR (Structure Medication Review) Screening (training can be given)
Person Specification
Qualifications
  • A relevant qualification to show you are registered Pharmacy Technician with GPHC
  • Have CPPE pathway completed
Experience
  • Have 6-12+ months experience working in primary care/GP practice setting
  • Have experience using SystemOne clinical system
