Qualifications

• Must understand the physiological, psycho-social, spiritual, and cultural needs of patients requiring specialist palliative care.
• Ability to complete assessments and identify individual patient problems.

Responsibilities

• Work closely with Clinical Leads to ensure patients achieve a peaceful death in their preferred place.
• Document and evaluate care, discussing progress with the multidisciplinary team.
• Assist patients and their families in preparation for changes in treatment.
