Job summary

We are seeking a compassionate and highly motivated Palliative Care Clinical Nurse Specialist to work within the Hospital Palliative Care Team. The role involves providing support and specialist palliative nursing input across the Trust, delivering patient-centred, safe and efficient specialist care in-line with the appropriate standards.

The post holder will:

  • Assess, plan, implement and evaluate the plan of care for palliative care patients
  • Provide specialist palliative care advice and education to clinical colleagues (multidisciplinary team) and patients/carers/relatives
  • Assist in the development of the service
  • Assist in the review and development of clinical guidelines, protocols and policies
Main duties of the job

Clinical -

  • Provide specialist nursing care and support the care provision for patients with specialist palliative care needs, utilising and developing referral and discharge criteria for specialist nurse intervention.
  • Perform comprehensive, holistic assessment of patients and carers using advanced skills in assessment, diagnosis and treatment. Work collaboratively with patients/families, colleagues and other professionals/services to ensure individuals\' palliative care needs are met.
  • Support and develop the continuity of care for patients throughout the whole pathway of care. Assist and/or advise on the nomination of a suitable professional as the key worker, as appropriate, at different points of the patients\' pathway.
  • Promote and provide effective communication with patients and carers and all clinical colleagues.
  • Maintain accurate, appropriate and concise patient records in line with the Trust's policy for record keeping.
  • Promote and provide relevant and clear information, education and advice for patients, carers and family according to their needs.
  • Act as the patient\'s advocate, as appropriate, supporting and promoting their involvement in decision making.
  • Provide specialist advice and information to professional colleagues, patients, carers and the public.
  • Work within the Airedale NHS Foundation Trust policies and procedures.
  • Work within all NMC code of conduct and practices.
