Job description

We currently have an exciting opportunity for an Onsite IP Clinical Pharmacist to work full-time at St Bartholomew's Practice within Oxford PCN. This role is suitable for a highly motivated and focused Clinical Pharmacist-IP, responsible for managing day-to-day activities within general practice, providing pharmaceutical care and advice to patients, and adhering to safety guidelines.

Full-time hours are within the core opening hours of the GP practice.

Main duties of the job

As an onsite Clinical Pharmacist-IP, you will confidently operate within professional boundaries, supporting and collaborating with a team of pharmacists and pharmacy technicians.

Your responsibilities include resolving medication-related queries, working as part of a multidisciplinary team, improving patient outcomes, and enhancing access to healthcare.

Job responsibilities

You will support and work alongside pharmacists and GPs, confidently acting within professional boundaries.

  • Conduct polypharmacy medication reviews and SMRs
  • Perform face-to-face medication reviews
  • Utilize skill mix effectively
  • Improve healthcare access
  • Assist practice staff with prescription and medication queries
  • Carry out medicine reconciliation and post-discharge reviews
  • Conduct medical audits and contribute to service improvement
  • Provide expert medication advice to clinicians and patients
  • Lead on medicines optimisation and quality improvement initiatives
  • Ensure the appropriate and efficient use of medications
  • Educate patients on medication use and disease prevention
  • Understand QOF, CQC, and local incentive schemes
  • Create and follow clinical pharmacy programmes according to practice policy
