Overview
We are looking for a passionate Nursing Assistant Practitioner to join the Norwich Community Learning Disability team in Norfolk. The service is a multidisciplinary team including community nurses, psychologists, occupational therapists, physiotherapists and speech and language therapists, integrated with the Norfolk County Council Learning Disability social work team. Nursing Assistant Practitioners are supported with clinical and managerial support and help deliver effective and efficient specialist health support for people with learning disabilities in the community under supervision.
Responsibilities
- Provide holistic and person-centred care for adults with learning disabilities across the lifespan in a variety of community settings, including people\'s homes.
- Work independently under the leadership of registered nurses, within all aspects of the nursing process.
- Apply the knowledge, understanding, skills, attitudes and behaviours relevant to employment as a Nursing Assistant Practitioner.
- Provide nursing care that is person-centred, safe and compassionate, supporting people to improve and maintain their mental, physical, behavioural health and wellbeing.
- Use evidence-based knowledge to make decisions and solve problems while maintaining professional conduct.
- Recognise and work within the limits of your competence.
- Engage in disease prevention and protection against ill-health.
- Participate in public health and community development and work to reduce health inequalities.
- Collaborate with health care professionals and statutory/non-statutory agencies to provide a seamless, integrated service to service users.
- Contribute to the delivery of care packages for patients with long-term conditions, rehabilitation and palliative care needs to achieve quality of life and independence where possible.
- Support early discharge from hospital and in-reach into inpatient units and mainstream services; help prevent unnecessary hospital admissions.
- Work with patients who have behaviours that challenge, mental health needs and autism spectrum conditions.
- Assess patients with straightforward requirements based on predetermined department protocols; prepare for, carry out and monitor assessments and interventions; discharge in line with protocols.
- Apply competency-based interventions under supervision, following a prescribed care plan; modify and progress intervention/treatment using clinical reasoning and notify a qualified practitioner as needed.
- Prepare for and develop individual or group activities to meet intervention needs; monitor patients and alert a qualified practitioner to any unexpected changes.
- Document visits and maintain records (e.g., input into PARIS) on a daily basis.
- Communicate effectively with patients and carers, ensuring explanations are accessible and respectful; establish and maintain professional relationships with colleagues and other health professionals.
Qualifications
- GCSEs Grade A-C in Maths and English or equivalent.
- NVQ 3 or equivalent knowledge and experience.
- Foundation degree programme or equivalent demonstrable knowledge and skills acquired through training and experience; previous experience in a health/care setting.
- Experience of working with people with learning disabilities.
- Experience of working with people in the statutory, voluntary or private sector.
Desirable criteria
- Experience of working in people\'s homes.
- Experience of working with people at the end of life; IT skills relevant to the role.
- Ability to work well in a team and on own initiative; ability to organise and prioritise workload.
- Able to recognise own limitations in the context of the nursing associate role; know when to seek advice and refer to a registered health or care professional.
- Participation in reflective practice and clinical supervision; awareness of multi-disciplinary team roles.
- Experience of working in a community setting; strives to understand each patient\'s total needs; contributes to team goals and works effectively with partners and agencies.
- Demonstrates high ethical standards and integrity with patients and peers.
- Must have a full UK driving licence and access to a vehicle.
