Job description

Peartree Surgery and West Horndon Surgery have a fantastic opportunity for a practice nurse to join our team ideally on a full time basis working 37.5 hours per week although this can be negotiable for the right candidate. Working across the two sites we have 3 highly supportive partners who will help develop skills with the candidate and try to further their career where possible. We are open to taking applications from newly qualified nurses to highly experienced nurses.

Responsibilities
  • Undertaking health assessments administering medication and treatments and performing diagnostic tests.
  • Providing and evaluating a range of health interventions to ensure that patients health and social care needs are met.
  • General health and well-being promotion as well as creating individual person-centred plans for patients with or at risk of a long term condition.
  • Managing and treating long-term conditions i n line with national and local policies and the practice needs.
  • Supporting and managing the health needs of women presenting for sexual health and contraception and cervical cytology consultations.
  • Implementing and participating in vaccination and immunisation programmes for both adults and children including patients travelling abroad (for NHS vaccinations only the practice does not operate private travel clinics).
  • Meeting the needs of patients presenting for opportunistic wound care.
  • Hypertension review and management (with clinical support).
  • Running asthma COPD and diabetes clinics and reviews (in-house support will be available for this)
  • New patient health checks (support and training will be provided).
  • Infection control leadership (training will be made available).
About the Candidate

All applicants should have the following qualifications registrations skills and experience :

  • A relevant nursing degree.
  • Current NMC registration with an active PIN.
  • Fundamentals of Practice Nursing course (General Practice Nurse Foundation Programme) or equivalent experience or qualifications.
  • Competence or ability to learn to undertake asthma COPD and diabetic clinics and reviews holding relevant certificates where possible (support will be provided).
  • Evidence of clinical skills and experience in treatment room consultation phlebotomy (optional) ECG recording (support will be provided) screening management of long-term conditions (support will be provided) cervical cytology sampling vaccinations and immunisations dressing and wound care.
  • Compassionate manner with an empathetic approach and demonstrable kindness in all aspects of your work. The successful candidate will value the dignity of our patients their relatives and carers as well as their colleagues at the practice.
  • Knowledge and experience of its application in supporting long-term conditions.
  • Insight and understanding of current issues in nursing and the wider NHS.
About Us

Peartree and West Horndon Surgeries are a patient focused and excelling health care provider for patients in South Ockendon and West Horndon localities. We currently consist of 7600 patients with our West Horndon site as a dispensing practice. We are constantly looking to improve our practices and take on new staff to help further our goal of great healthcare. We try to work in a family type environment and look after our staff and colleagues. Our surgery has 3 partners and one nurse. Our dispensing team consists of 4 pharmacy dispensers.
