New to Practice Nurse (Northern Parishes)

An exciting opportunity has arisen for a New to Practice Nurse in Primary Care. West Lancashire GP Federation is looking for a Practice Nurse to join Northern Parishes PCN.

As a newly qualified / new to General Practice Nurse, you will be supported through a preceptorship to build your confidence and expertise in Primary Care as you transition from training or another sector. You will work under the guidance of an experienced team including GPs and Nurses as you grow in your role.

Main duties of the job

The role is patient facing providing nursing led care to patients. As you progress through your preceptorship, your role will develop and evolve to include:

  • Early detection, prevention and management of cardiovascular disease and other long term conditions
  • Taking a lead role in managing a specific Long Term Condition, following structured mentorship and Preceptorship.
  • Participating in quality improvement initiatives to enhance patient outcomes and reduce health inequalities
  • Working as part of a multidisciplinary team, supporting integrated care and personalised approaches to patient care
About us

Employment will be with Out of Hours West Lancashire CICLimited (OWLS). We host roles on behalf of the three West Lancashire PrimaryCare Networks (PCNs) and their member practices.

OWLS is a small GP owned and led not-for-profit primary careorganisation run by GPs and health professionals. As a not-for-profitorganisation all the money we generate through service contracts is reinvestedin providing patient care.

We were founded in the 90s by a small group of GPs toprovide high quality out of hours services. In 2017, we became the WestLancashire GP Federation. The Federation supports and provides servicesdirectly and with partners, for West Lancashire GP practices and provides avehicle to bid for and provide primary care services.

Northern Parishes PCN brings together five GP practices, Burscough Family Practice, Lathom House, Stanley Court Surgery, Parbold Surgery and Tarleton Family Practice,with a combined list size of 29,796.

Job responsibilities

As a newly qualified / new to General Practice Registered Nurse, you will be supported through your Preceptorship, where you will receive 12 months of expert support, training, and supervision to help you grow into a confident, autonomous professional, where subject to training and sign off your work will include:

  • Undertake phlebotomy and interpret blood results
  • Administer routine injections and vaccinations, such as flu shots and childhood immunisations, with support from senior staff
  • Support patients with Long Term Conditions including Diabetic Reviews, management and foot checks
  • Perform basic ECG tests as needed, under supervision, to support patient diagnosis and monitoring.
  • Perform Spirometry and FENO tests and interpret results
  • Carry out Dementia, Frailty and Learning Disability Reviews
  • Support patient education around lifestyle choices such as diet, exercise, smoking cessation, and overall health and promote preventative care through health and promotion activities.
  • Carry out cytology screening including smears
  • Perform blood pressure checks and basic cardiovascular monitoring as part of regular patient reviews and document results in patient records
