Job description

The post-holder will be a member of the Neighbourhood Nursing (District Nursing) Team and will participate in the clinical case management of patients with a wide variety of complex, high-risk and supported/self‑care conditions. Working directly with the Team Leader, the post-holder will help ensure the provision of a high standard of nursing care and health promotion. Care will be provided in a range of settings including patients' homes, residential homes, nurse‑led clinics and GP practices.

Responsibilities
  • Holistic assessment, planning, implementation and evaluation of a plan of care for patients with long‑term conditions, tissue viability, continence, palliative care, and promoting and facilitating patient independence.
  • Assist in the delivery of National Service Frameworks.
  • Provide public health and health promotion programmes to improve health and reduce inequalities.
  • Maintain contemporaneous written and computerised records.
