A leading healthcare provider is seeking experienced Band 5 nurses to join Neighbourhood Nursing teams in Lambeth and Southwark. The role involves holistic assessments, planning care for patients with complex conditions, and promoting health in various settings including homes and clinics. Candidates must have significant community nursing experience and a commitment to providing high-quality care. The position offers flexible working opportunities.
The post-holder will be a member of the Neighbourhood Nursing (District Nursing) Team and will participate in the clinical case management of patients with a wide variety of complex, high-risk and supported/self‑care conditions. Working directly with the Team Leader, the post-holder will help ensure the provision of a high standard of nursing care and health promotion. Care will be provided in a range of settings including patients' homes, residential homes, nurse‑led clinics and GP practices.
