Job description
Overview

The Neighbourhood Coordinator will work with the Head of Integrated Neighbourhood Working and Senior Neighbourhood Care Coordinator, member practices and key local operational stakeholders to support coordination of care for patients through a multidisciplinary team meeting approach, stakeholder signposting and supporting the across the whole South West footprint. Our Neighbourhood Team Coordinator posts are an exciting new addition to our already existing model of local population based health and care. The roles will support our Primary Care Networks in the Sleaford and Grantham localities of South West Lincolnshire, being aligned to our established, integrated Neighbourhood Team.

Requirements

We are looking for individuals with proven experience in health and care environments or equivalent experience and evidence of continued personal development. You will also have excellent interpersonal and communication skills, be highly motivated and positive in your approach to care delivery to ensure we maintain a high quality service delivery to our population. Do you share the Lincolnshire Health and Care Home First principles? Do you have experience of working within health and care services and are looking for a new and exciting challenge? If so we are looking for individuals who champion local high quality integrated care and support to play a key role in positive change in our local community.
