Mobilisation Nurse - Complex Care (South London)

£45,000 - £50,000 per annum | Full-time | Permanent

Are you a Registered Nurse with a passion for delivering safe, high-quality care in the community? We're looking for a Mobilisation Nurse to lead the setup of new complex care packages across South London, ensuring a smooth, person-centred transition for clients and their families.

Working for a respected complex care provider, you'll play a key clinical role from day one — planning, assessing, and overseeing the mobilisation of paediatric and adult care packages in the home. It's an exciting opportunity for a nurse who thrives on autonomy, impact, and clinical excellence.

Responsibilities
  • Carrying out holistic clinical assessments and creating safe, responsive mobilisation plans
  • Overseeing the onboarding process, including carer training, competency reviews, and in-home oversight
  • Working closely with families, CHC teams, case managers, and internal staff
  • Providing visible support during the first 4 weeks of each new care package
  • Representing the provider at multidisciplinary meetings to support service growth
Qualifications
  • NMC Registered Nurse (Adult or Children's)
  • Background in community, complex, or continuing care
  • A confident communicator who thrives on planning, coordination, and delivering safe outcomes
  • Flexibility to travel across South London and occasionally outside 9-5 hours
  • Full UK driving licence and access to your own vehicle
