A leading healthcare organization in England is seeking a full-time Health Care Support Worker to support individuals with complex mental health challenges during their transition to community-focused care. The role involves providing practical and emotional support, engaging effectively with service users, and facilitating access to community resources. Ideal candidates will have a foundation degree in health-related fields and significant experience in mental health settings.
Band 4 Health Care Support Worker within Integrated Mental Health Teams (IMHT) across South Staffordshire, supporting the First Transitions Pathway. The First Transitions Pathway supports individuals with complex mental health as they move from forensic services to community-focused care, promoting sustained recovery and meaningful integration within the community.
You will provide practical and emotional support directly to service users during the immediate transition and in the weeks and months that follow, building rapport, engaging effectively, and facilitating access to community resources.
