Responsibilities

  • Conduct comprehensive initial memory assessments for individuals with cognitive difficulties.
  • Implement person-centred approaches, encouraging engagement with the Memory Assessment Service.
  • Utilize a risk assessment framework to ensure collaboration and safeguarding.
Job description
  • Carry out comprehensive and specialised initial memory assessments for individuals experiencing memory or cognitive difficulties that may be associated with dementia.
  • Apply person-centred and compassionate approaches, actively listening and responding to individuals to encourage their engagement with the Memory Assessment Service (MAS).
  • Use a robust framework for risk assessment, safeguarding, signposting, and referrals to specialist teams, ensuring collaboration with the individual throughout the process.
  • Provide support across Derbyshire, delivering services in a variety of settings to meet the needs of the Memory Assessment Service.

  • Our service runs Monday to Friday, 9:00am-5:00pm (excluding bank holidays). This is a full-time, permanent position of 37.5 hours per week.
