We have an exciting opportunity for Bank staff Anticoagulation services for clinics across four different broughs:

Waltham Forest

Havering

Newham

WalthamForest Federated GP Network are recruiting IP Clinical Pharmacists. We reserve the right to amend the advert depending on the needs of the business and also to withdraw the advert upon recruiting staff.

The Clinical Pharmacist willhave a key role in supporting delivery of the new Network Contract DES Servicespecifications. The new Structured Medications Review and Optimisationrequirements will include tackling over-medication of patients, includinginappropriate use of antibiotics, withdrawing medicines no longer neededthrough NHS-led programmes such as low priority prescribing and medicinesoptimisation more widely. For Enhanced Health in Care Homes residents willbenefit from regular clinical-pharmacy-led medicines reviews. The clinics will be based in practices and there are home visits too.

Main duties of the job
  • Work as part of a multi-disciplinary team in a patient-facingrole to clinically assess and treat patients using their expert knowledgeof medicines for specific disease areas.
  • You may be expected to work in different boroughs

  • Uphold a prescribing qualification , and will work with and alongside themulti-disciplinary team across the Primary Care Network.
  • Take responsibility for the care management ofpatients with chronic diseases and undertake clinical medication reviewsto proactively manage people with complex poly-pharmacy, especially theelderly, people in care homes, those with multiple long-term conditions(in particular COPD and asthma) and people with learning disabilities orautism (through STOMP - Stop Over Medication Programme).

Job responsibilities
  • Work as part of a multi-disciplinary team in a patientfacing role to clinically assess and treat patients using their expertknowledge of medicines for specific disease areas.
  • Work in anti-coagulation clinics potentially across different boroughs
  • Uphold a prescribing qualification , and will work with and alongside themulti-disciplinary team across the Primary Care Network.
  • Take responsibility for the care management ofpatients with chronic diseases and undertake clinical medication reviewsto proactively manage people with complex poly-pharmacy, especially theelderly, people in care homes, those with multiple long term conditions(in particular COPD and asthma) and people with learning disabilities or autism(through STOMP - Stop Over Medication Programme).
  • Provide specialist expertise in the use of medicineswhile helping to address both the public health and social care needs ofpatients in the network.
  • Provide leadership on person centred medicinesoptimisation (including ensuring prescribers in the practice conserveantibiotics in line with local antimicrobial stewardship guidance) andquality improvement, while contributing to the quality and outcomesframework and enhanced services.
  • Through structured medication reviews, supportpatients to take their medications to get the best from them, reduce wasteand promote self-care.
  • Support further integration of general practice withthe wider healthcare teams (including community and hospital pharmacy) tohelp improve patient outcomes, ensure better access to healthcare and helpmanage general practice workload.
  • Develop relationships and work closely with otherpharmacists across networks and the wider health system.
  • Take a central role in the clinical aspects of sharedcare protocols, clinical research with medicines, liaison with specialistpharmacists (including mental health and reduction of inappropriateantipsychotic use in people with learning difficulties) and anticoagulation.

  • Participatein regular supervision sessions and appraisals with your line manager. Attend training and development activities asidentified and participate in meetings as required.
  • Maintainadministration systems and workspace used in a clean and tidy condition.
  • Maintaina good working knowledge of Health and Safety procedures and fire precautions,and operate the correct procedures and participate in policy development anddata collection where appropriate.
  • Workflexibly to meet the needs of clients.
  • Toundertake any other duties appropriate to the grade and purpose of the job asmay be agreed by the post holder.
Person Specification
Qualifications
  • Must be an Independent Prescribing Pharmacist
  • Independent prescriber but also with completed ACP
  • Desired start date to be as soon as possible
Experience
  • Independent prescriber
  • Minimum of two years experience as a pharmacist, demonstrated within a practice portfolio
  • Experience and an awareness of the breadth of common acute and long-term conditions that are likely to be seen in general practice
