Hybrid Clinical Pharmacist-IP Nottingham (TPN-HYB-CPIP-NOTTS)
As a clinical pharmacist-IP, you will:
- Support and work alongside a team of pharmacists and GPs, acting confidently within professional boundaries.
- Resolve medication-related queries, conduct polypharmacy medication reviews, and face-to-face medication reviews.
- Help to utilise skill mix and improve patient outcomes.
- Ensure better access to healthcare and support practice staff with prescription and medication queries.
- Perform medicine reconciliation and review post-discharge.
- Conduct medical-related audits and work on service improvement within the practice.
- Provide expert advice on medications to clinicians and patients.
- Deliver clinical leadership on medicines optimisation and quality improvement.
- Ensure the appropriate and efficient use of medication.
- Educate patients on medication and disease prevention.
- Understand QOF, CQC, and local incentive scheme workstreams.
- Create and follow a clinical pharmacy programme according to practice policy.
