Hybrid Clinical Pharmacist-IP Nottingham (TPN-HYB-CPIP-NOTTS)

As a clinical pharmacist-IP, you will:

• Support and work alongside a team of pharmacists and GPs, acting confidently within professional boundaries.
• Resolve medication-related queries, conduct polypharmacy medication reviews, and face-to-face medication reviews.
• Help to utilise skill mix and improve patient outcomes.
• Ensure better access to healthcare and support practice staff with prescription and medication queries.
• Perform medicine reconciliation and review post-discharge.
• Conduct medical-related audits and work on service improvement within the practice.
• Provide expert advice on medications to clinicians and patients.
• Deliver clinical leadership on medicines optimisation and quality improvement.
• Ensure the appropriate and efficient use of medication.
• Educate patients on medication and disease prevention.
• Understand QOF, CQC, and local incentive scheme workstreams.
• Create and follow a clinical pharmacy programme according to practice policy.
