Responsibilities
  • As a key member of our care team, you’ll work under the supervision of a Case Manager to deliver direct personal care and support to patients at home.
  • You’ll help patients with daily living activities, provide companionship, and monitor their condition—while keeping the care team informed every step of the way.
Responsibilities – Day-to-Day
  • Assisting patients with bathing, dressing, grooming, and personal hygiene
  • Helping with toileting needs and mobility support
  • Performing light housekeeping duties, changing linens, and preparing meals
  • Reporting food and fluid intake/output as assigned
  • Taking and recording vital signs under professional supervision
  • Providing medication reminders and emotional support
  • Observing and reporting any changes in patient condition or behavior
  • Documenting care accurately and communicating with the care team
  • Supporting therapy exercises as directed
  • Calling to confirm scheduled visits and respecting each patient’s routine
