As a Highly Specialist Pharmacist, you will be responsible for providing a highly specialized clinical pharmacy service to the agreed specialty, including:
Taking drug histories, completing medicines reconciliation, and screening patients for medication-related issues promptly after admission.
Following up with patients post-discharge, ensuring communication of changes in medications to the GP, and patient awareness of any changes.
Assessing patient needs for medication, determining optimal choices, and effectively communicating potential interactions, contraindications, or cautions to clinicians.
Providing highly specialized pharmaceutical advice to clinicians, healthcare professionals, and patients, both within the Trust and externally as needed.
Demonstrating advanced levels of clinical reasoning and judgment, utilizing persuasive skills when communicating with medical staff, and ensuring consultants are informed about evidence underpinning recommended treatment choices.
