Overview

Heart failure doesn’t mind your age or appearance, and it is on the rise. This is a great opportunity to join a dynamic team exploring ways of supporting patients living with heart failure across the Vale of York. We have been piloting new ways to improve management of heart failure in both the acute setting and the community, with a view to shaping the future of heart failure nursing management. This post will allow us to expand and develop the services we offer, providing the chance to be involved at the front of an innovative service and to progress knowledge and skills in heart failure while working with committed colleagues.

Support will be provided to work towards the heart failure specialist nurse competency framework set by the British Society of Heart Failure.

Responsibilities
  • Provide high quality expert care to patients with heart failure in York hospital and the Vale of York community setting as part of a team of experienced heart failure specialist nurses.
  • Contribute to the heart failure specialist nurse team delivering an out-of-hospital rapid access clinic at a local health centre in central York, community clinics across the Vale of York, and home visits to housebound patients.
  • Help develop and implement an in-reach model focused on admission avoidance and early step-down to community care for patients managed at York Hospital.
  • Engage in the ongoing development of services to improve management of heart failure patients and participate in change initiatives.
Requirements / Qualifications
  • Ability to transport yourself and equipment across the locality during a normal working day; access to a vehicle for business purposes and a full valid driving licence is required (travel to multiple addresses not easily reached by public transport).
  • Working towards or undertaking the heart failure specialist nurse competency framework as set by the British Society of Heart Failure.
  • A willingness to work within a front-line innovative service and collaborate with colleagues across hospital and community settings.
