Overview

Healthcare Assistant (Community based)
Location: Nottingham (NG8 6AT) with travel around the area
Salary: £22,932 - £23,304 per annum
Hours: 35 per week
Contract: Fixed term contract (31st December 2025)
Driving: Full UK Driving Licence and access to a vehicle

Responsibilities
  • Providing invaluable contribution to the Community Crisis Support Service and its users.
  • Providing short-term crisis care in people\'s homes to help prevent hospital and care home admissions, supporting individuals to remain safely in their communities across Rushcliffe, Nottingham North East, and Nottingham North West.
  • Assessing clients\' needs and develop a care plan to fit them.
  • Providing support with personal care, medication prompts, end of life care, light meal and drinks preparation.

Qualifications & Requirements
  • NVQ Level 2 or equivalent in health and social care
  • Ability to communicate with health and social care professionals
  • Ability to deal with service user queries in a diplomatic, professional and confidential manner
  • Knowledge of assisting service users, who may be disabled and/or frail, cognitively impaired
  • Good geographical knowledge of the service catchment area
  • IT literate
  • Knowledge of services provided by the NHS and Social Care
  • Good working experience of the delivery of social care support including personal care, feeding and medication prompting
  • Participation in a multi-disciplinary team environment
  • Full driving licence holder and access to vehicle, ability to use the vehicle daily for service user visits
  • Willingness to work flexible hours
