Job description

To plan, deliver and evaluate an efficient and cost-effective Health Visiting service for an identified population. To provide leadership and line management of skill mix within the Health Visiting team, and be pro‑active in delivering the National Healthy Child Programme Public Health and Family Hubs agendas in response to identified health and social need. To work in partnership with other agencies and meet agreed standards, objectives and statutory requirements.

Leading and delivering the Healthy Child Programme, to be universal in reach and personalised in response to address the 6 early years high impact areas. Leading strengths‑based approaches across the levels of need at community, universal, targeted and specialist intervention, reducing health.

Hold a caseload of children/ families until the end of Early Years foundation stage (age 5 years), and beyond where working with families at targeted and specialist level delivering enhanced family support. Be responsible for effective caseload/workload management, providing specialist evidence‑based advice, information and support to the population served, team members and relevant agencies.
