Overview

Working with service users referred to Health & Justice and with their carers, helping to promote self-determination, choice and autonomy. To effectively establish a therapeutic relationship with service users and relatives/carers.

Responsibilities
  • Participate in setting quality standards, including auditing, monitoring and reviewing practice in line with current clinical guidance, practice and policy.
  • Assess patients' individual care needs, agree and refer on to the appropriate agency.
  • Involve the individual in the planning and prioritisation of their activity where possible.
  • Provide short-term interventions to encourage individuals to engage with services and attend their initial appointments.
