Main duties of the job

You will be part of a MDT team in which you will receive clinical and managerial support, which is also part of a wider MDT team that is aligned to social care professionals in reflection of the NHS 10 year plan.

You will be responsible for delivering quality care to a designated caseload of people in the place that they call home, you will be following a care plan devised by the community nursing staff who you will be working alongside. You will be asked to feedback concise information in a daily handover to ensure changing needs are met and risks escalated.

Job responsibilities

The successful candidate will be expected to be flexible and assist colleagues to cover service needs, which will include working across the wider integrated team. Access to a car in order to carry out service duties is essential.

A full induction programme will be provided.

We have posts available to work alongside the Biggleswade community nursing team delivering care to people living in Biggleswade, Sandy, Potton and surround villages and the Ampthill Community nursing team, delivering care to people in Flitwick, Barton and Woburn and the surrounding Mid West Beds villages.

The Community Nursing Service operates between 08.00 to 21.00 hours and the working pattern will include a variety of rotational shifts which will include weekend, evening and Bank Holiday working. Part time posts would be considered.

Person Specification
Education/Qualification
  • NVQ level NVQ level 2 and / or significant care experience and willing to work towards NVQ Level 32 and / or significant care experience and willing to work towards NVQ Level 3
  • Completed a recent Care Certificate
  • Educated to GCSE or equivalent and hold
Knowledge and Skills
  • Literate in IT/Computer Skills
  • Good communication skills both verbal and written
  • Rehabilitation skills
  • The ability to collect and collate data
  • Deliver high quality care in a variety of settings some of which may be unpredictable or undesirable.
  • Understand safeguarding and manage risk
Other
  • Undertake the moving and positioning of patients and equipment adhering to the manual handling policy when in clinical practice.
  • Current driving licence
  • Ability to travel across sites and across Trust boundaries to meet patient needs and attend meetings, etc.
  • Good oral communication skills based on fluency on the English language
Experience
  • Working in a Community Health or Social Care setting
