General Practice Nurse

The post holderis responsible for ensuring the delivery of safe and effective nursing care tothe whole practice population. As an autonomous practitioner the nurse is responsible for delivering patient care, supporting patients to be healthy, monitoring of long-term conditions, health promotion and screening activities and demonstrating critical thinking and skills in clinicaldecision-making. They will work collaboratively with the whole general practiceteam to meet the needs of patients, supporting the delivery of policy andprocedures.
Main duties of the job
- Prescribe medication within own scope of practice
- Administer appropriate vaccines to adults and children according to up to date Department of Health guidelines including vaccinations for foreign travel
- Review medication for effectiveness, need and national guidance
- Support patients to ensure compliance with prescribed medications
- Family Planning advice and help including checking of caps and IUCDs
- Cervical cytology
- Giving health promotion advice to patients during consultations as appropriate
- Monitoring of long term conditions
- Wound / ulcer care
- Removal of Sutures
- Urinalysis
- ECG recording
- Blood Pressure measurements
- Asthma/Peak Flow Readings/Spirometry
- Venepuncture
- Assisting GPs with minor surgery and coil fittings
- Requesting pathology tests, for example urine culture, swabs
- Maintaining stocks of medicines, drugs, vaccines, dressings and equipment and re-ordering as necessary
- Liaison with other agencies, including Acute and Community Trusts
- Ensure consultations and information are recorded in the computer records as appropriate
- Attend and participate in any Practice meetings when required
- Able to recognize and manage anaphylaxis according to current UK guidelines
- Able to perform cardio-pulmonary resuscitation according to current UK guidelines
- Any other delegated duties appropriate to the post

Job responsibilities
- To ensure the highest standards of care areprovided for patients
- To manage and develop a clinical acute andchronic case load, dealing with presenting patients needs in a primary caresetting
- Review medication for therapeuticeffectiveness, appropriate to patients needs and in accordance withevidence-based practice and national and practice protocols.
- Prioritise health problems and interveneappropriately to assist the patient in complex, urgent or emergency situations,including initiation of effective emergency care
- To deliver a high standard of patient careusing specialist practice Nurse skills
- Attend and contribute to relevant internal andexternal meetings as required including practice clinical meetings, nurse teammeetings and gold standard framework meetings
- Maintain a personal progress and training planto keep up-to date with current thinking and developments as well asidentifying own training deficiencies and attending suitable courses asappropriate
- Promote evidence based practice through use ofthe latest research based guidelines
- Monitor the effectiveness of their ownclinical practice through quality assurance strategies such as the use of peeraudit and review
- Maintain professional registration
- Work within the latest NMC Code ofProfessional Conduct
- Ensure collection and maintenance ofstatistical information required for regular and ad hoc reports.
- Record accurate consultation data in patientsrecords in accordance with NMC guidance and other standards
- Keep up to date with pertinent health policyand work with the practice team to consider the impact and strategies forimplementation
- Work collaboratively with colleagues bothinternally and externally
- Use technology and appropriate softwarepackages as an aid to the planning, implementation and monitoring of care,presenting and communicating information
- Act as a role model in the observance ofequality and diversity good practice
- Consultingfor minor health problems in practice nurse minor illness surgeries andobtaining prescriptions where necessary. The On Call Team will always beavailable for advice and will see patients if necessary

Person Specification
Qualifications
- Qualified and Registered Nurse.
Personal Qualities
- Team player with excellent communication skills able to follow protocols, working with the team to improve protocols as necessary.
Experience
- Experience of working within a Nursing Team
- Experience of working in primary care
- Experience of working in a GP practice
- These are desirable as for the right candidate, we are happy to provide the training needed for nurses who are currently working in the community teams or secondary care.
