Summary
Alert pharmacy technician coach to any issues compromising the quality of projects and operational workstreams.
Effectively manage own time, workload, and resources.
Support training and development of administrative staff to support pharmacy service in line with protocols and procedures.
Work within risk management and health and safety policies and procedures within GP practices and other providers.
Adhere to a robust process for recording learning events, complaints, accidents, and significant events.
Support the development and management of any changes or new processes and systems that need implementing.
Carry out any other appropriate duties as determined by the organization.
Should hold GPhC registration.
Job Summary
Job Purpose:
Provide technical support to the clinical pharmacist and other pharmacy services within general practice and the Primary Care Network (PCN) to ensure professional operational standards and excellence in pharmaceutical care and safety are maintained.
Ensure delivery of safe, effective, and efficient systems for medicines-related clinical correspondence, repeat prescribing, medicines optimisation, reducing medicines waste, and maximising patient outcomes.
Main Duties of the Job
Undertake patient-facing and patient-supporting roles to ensure effective medicines use through shared decision-making conversations with patients.
Carry out medicines optimisation tasks including effective medicine administration (e.g., checking inhaler technique), supporting medication reviews, and medicines reconciliation. Use consultation skills to work in partnership with patients to ensure proper medicine use.
Support medication reviews and medicines reconciliation for new care home patients, and synchronise medicines for patient transfers between care settings, liaising with local pharmacists.
Provide expertise to address public health and social care needs of patients, including lifestyle advice, service information, and tackling health inequalities.
Participate in shared care protocols and liaise with specialist pharmacists for complex cases.
Support antimicrobial stewardship initiatives to reduce inappropriate antibiotic prescribing.
Assist in medicines optimisation schemes and patient safety audits.
