The role requires working autonomously within a community setting to provide acute assessments and patient support. Ideal candidates will have significant clinical experience and a commitment to high-quality patient care, with strong teamwork skills. Competitive compensation based on experience is available.
The East Lancashire Alliance are currently looking to recruit a Paramedic Practioner on behalf of the Burnley East PCN.
The successful candidate will be responsible for overseeing the clinical care of our ageing population. You will be a highly experienced, senior clinician who works closely with patients (mainly those with a long-term condition or complex range of conditions) in their place of residence. You will be able to directly provide, plan and organise their care, working with the wider Primary Care Network team to ensure the highest level of care for the patients. The post-holder will work autonomously alongside the GP Practices in Burnley East Primary Care Network, acute care teams and within the community at an advanced level of practice, using their enhanced clinical assessment and treatment skills, to provide first point of contact for patients presenting with undifferentiated, undiagnosed problems. The interface between General Practice and other community services care homes, social services, community nursing teams etc will be a key area of development as we look to move from reactive to proactive care. The post-holder will have a role in developing these relationships and pathways.
