Experienced Paramedic Practitioner Burnley East PCN

The East Lancashire Alliance are currently looking to recruit a Paramedic Practioner on behalf of the Burnley East PCN.

The successful candidate will be responsible for overseeing the clinical care of our ageing population. You will be a highly experienced, senior clinician who works closely with patients (mainly those with a long-term condition or complex range of conditions) in their place of residence. You will be able to directly provide, plan and organise their care, working with the wider Primary Care Network team to ensure the highest level of care for the patients. The post-holder will work autonomously alongside the GP Practices in Burnley East Primary Care Network, acute care teams and within the community at an advanced level of practice, using their enhanced clinical assessment and treatment skills, to provide first point of contact for patients presenting with undifferentiated, undiagnosed problems. There is a strong focus on teamwork, working with the wider team to support the rapid assessment of same-day presentations. The interface between General Practice and other community services care homes, social services, community nursing teams etc will be a key area of development as we look to move from reactive to proactive care. The post-holder will have a role in developing these relationships and pathways.

Main duties of the job
  • To have knowledge and skills pertaining to the relevant specialist area.
  • To have knowledge and understanding of the challenges of caring for people in the place of residence.
  • To work with clinical and non-clinical team members to identify unmet needs of the ageing population.
  • Provide acute assessments to reduce hospital admissions including but not limited to.
  • Take detailed comprehensive patient history.
  • Carry out physical examinations.
  • Use expert knowledge and clinical judgement to identify potential diagnosis.
  • Refer patients when needed o Decide on and carry out treatment, including the prescribing medicines.
  • Ensure the provision of continuity of care, including follow-up visits.
  • Use shared decision making to aid the creation and development of personalised care and support plans.
  • Work with the multi-agency teams to provide integrated care for patients.
  • Provide anticipatory care planning.
  • Making sure patients get quality care.
  • Empowering others to take on a wider range of tasks.
  • Delivering services in line with service specifications and national requirements.
  • Making sure service users are treated with respect.
  • Preventing avoidable admission.
  • Resolving problems for service users and their relatives by building closer relationships.
  • To work closely with PCN member practices to meet the needs of their aging population.

Qualifications
  • BSc in a training programme approved by the College of Paramedics
  • Health & Care Professions Council (HCPC) registration
  • able to operate at an advanced level of clinical practice
  • Framework for Higher Education Qualification (FHEQ) Level 7 or Scottish Credit and Qualifications Framework (SCOF) Level 11
  • Level 3 Safeguarding (Children)
  • Full UK driving license
