Job Overview

As a Community Support Assistant, you will be part of our Community Diabetes Team, working in various clinical areas across Liverpool, including local communities, Primary Care Networks, and patient homes. The role supports the Clinical Diabetes Team, delivering education and training on glucose monitoring devices and diabetes management.

Key Responsibilities
  • Ensure appropriate prioritisation of work in collaboration with the Diabetes Specialist Nurse.
  • Competently handle the application, support, removal and data downloading of glucose monitoring devices.
  • Provide support to patients using glucose monitoring devices, including education sessions for individual or group patients, via virtual and face‑to‑face methods.
  • Deliver education sessions to people with diabetes, teaching them to use glucose sensors and related technologies.
  • Support the Clinical Diabetes Team and undertake clinical responsibilities under the Diabetes Specialist Team direction.
  • Maintain supplies, implement back‑up systems when electronic systems fail, and maintain equipment, performing internal quality control checks.
  • Set up, maintain and download glucose monitoring systems, sharing information with medical and nursing staff.
  • Assist in providing health care for patients within the Team.
Qualifications and Competencies
  • Assistant Practitioner Qualification (Health).
  • Basic Life Support and previous healthcare experience, including experience in phlebotomy/venepuncture.
  • Computer literate, using Microsoft Excel, Electronic Patient Record and other systems needed for downloading and interpreting monitoring data.
  • Awareness and understanding of the Diabetes Technician role as per the NHS Plan and recent diabetes initiatives.
  • Ability to organise and manage own workload.
  • Excellent communication skills; able to relate and communicate with patients and staff at all levels and from different professions; adapts communication style as necessary and works collaboratively.
  • Ability to use technology proficiently and to teach others how to use it.
