A community support organization is seeking a Community Support Worker to assist adults transitioning from hospital to home. The role emphasizes prevention and support within the community, requiring strong relationship-building skills and knowledge of social care legislation. Candidates should have relevant experience and an NVQ/BTEC Level 3 qualification or equivalent. This position offers a fixed-term contract and requires mobility across Essex for service user transport.

£25,395 to £32,131 per annum

Enabling people to access information and tools, live independently and respond positively to growing service demand sustainably within the financial constraints of ECC.
The Integrated Neighbourhood model of practice is ever evolving and developing. Working within one of the Mid Locality Teams you will be an influencer of change and progression. The ethos is to support adults’ prevention from hospital and with discharge from hospital, when safe and appropriate to do so, and to continue their care and assessment out of hospital. Where possible adults will be supported to return to their home for assessment, remaining as independent and self‑caring as possible. Adults discharged to a temporary care home environment will be assessed to identify short‑ or long‑term care and support needs.
The role requires you to be mobile throughout a wider operational area, including travelling across Essex and beyond, transporting service users and their belongings, and to be able to do so in an emergency. Driving licence and car or alternative means to meet the mobility requirements is essential.
As part of employment with Essex County Council (ECC) if your role requires a DBS check you will be required to register with the DBS update service.
