Community Support Team (CST) Peer Support Professional (2195)

We are seeking a full‑time Peer Support Professional to join our Community Support Team (CST). In this role you will provide clinical services to all clients receiving CST team services, including tenancy support interventions using the Permanent Supportive Housing model; service coordination; symptom assessment and management; individual supportive counseling; activities‑of‑daily‑living social interpersonal relationship and leisure‑time‑activity services; support services or direct assistance to ensure that clients obtain the basic necessities of daily life; and education support and consultation to individuals, families and other major supports.

Compensation

This position is compensated using a piece rate model where pay is based on the Relative Value Units (RVUs) assigned to each billable service. Each service is assigned a specific RVU value and your total compensation is determined by the number and type of services you deliver.

  • Orientation rate: $17/hourly
  • Piece Rate: $22 per RVU – $24.50 per RVU
Key Responsibilities
  • Assist in identifying, planning, scheduling and implementing services for the client.
  • Assist in the development of transition plans when clients move in and out of services.
  • Act as an advocate in the community.
  • Provide written documentation as required, including treatment notes, goals and plans for the medical record (e.g., service notes input into person‑centered plan).
  • Be available to respond as needed to clients and co‑workers 24/7/365.
  • Must be free from all illicit substance use.
  • This is considered Light Work; no lifting over 20 pounds.
  • Demonstrate adherence to the treatment models within CST such as Permanent Supportive Housing, CBT strategies, MI and Harm Reduction.
  • At the discretion of the team lead or supervisor and/or management, this position may require additional duties not specifically listed above.
Qualifications
  • Must have completed a certification program for Peer Support Specialists and have at least two years of sustained recovery from any identified mental health or substance use issues.
  • Must become NVCI certified and have received training in the population served and client rights/confidentiality.
  • Valid driver’s licence and current automobile insurance required.
