About the Role

Lincolnshire Community Health Service (LCHS) is looking for Registered Nurses to join their Sleaford Community Nursing Team.

Responsibilities
  • Work autonomously and as part of a team delivering care to patients in their homes, care homes, and clinic settings.
  • Assess care needs and plan, deliver and evaluate nursing care using evidence‑based practice.
  • Provide wound management, continence care, palliative care, medication administration and long‑term condition management.
  • Collaborate with the multidisciplinary team (social workers, GPs, allied health professionals) to ensure coordinated care.
Qualifications and Experience
  • Registered Nurse with valid registration.
  • Experience in community or ambulatory nursing preferred but not mandatory.
  • Ability to travel to patients' homes and clinic settings; personal transport is essential due to rurality.
Working Hours & Induction

Hours: 8:00 am – 6:30 pm, 7 days a week, with variable shift patterns – Early (8:00 am – 4:00 pm, 9:00 am – 5:00 pm) and Late (10:30 am – 6:30 pm).

12‑week induction programme and robust preceptorship with a clinical education team.
