Overview

Community Staff Nurses

Out of Hours 18:00-23:00 • Two evenings a week

All candidates must have a clean UK driving license and use of a car for work purposes.

We are Transforming Adult Community Services in Croydon. The Croydon Alliance Board aims to provide a whole system transformation to deliver outcomes requested by people over 65. The Outcome Based Commissioning programme is a radically different approach to funding and delivery of services designed to deliver outcomes that local people want while getting best value from Health and the Social Care sector. This includes well coordinated care and support to promote independence in the most appropriate care setting.

We have exciting opportunities for highly motivated, enthusiastic Registered Adult Nurses to work within our Integrated Care Teams. We offer a comprehensive induction programme, preceptorship, Clinical Supervision and excellent ongoing professional development opportunities for enhanced career progression. Once recruited staff can take advantage of our child care facilities.

Main Duties and Responsibilities
  1. Promote the mental, physical and emotional and social health of individuals, families and groups.
  2. Provide a high standard of care to individuals and families.
  3. Be aware of the needs of patients and their relatives and have knowledge of statutory and voluntary services.
  4. Be an effective member of a Nursing Team working within the community directorate.
  5. Be responsible for caseload management and supervision of the nursing team in the absence of the District Nurse/ Caseload Holder.
