Overview

As a community staff nurse you will be required to work autonomously and as part of a team, delivering care to patients in their own homes, care homes and clinic settings. Successful candidates will be responsible for assessing care needs, and the planning, delivery and evaluation of nursing care using innovative evidence based practice. Community nursing is exceptionally rewarding as it allows wonderful therapeutic relationships to be developed with patients in their own homes. Working in this capacity embodies the person centred care approach and allows patients to truly be at the centre of care delivery.

Responsibilities
  • Assess care needs and plan, deliver and evaluate nursing care using evidence based practice.
  • Provide interventions such as wound management, continence care, palliative care, medication administration and long term condition management.
  • Work closely with the MDT (Social Workers, GPs and Allied Health Professionals) to ensure coordinated patient care and positive impact on patients.
