Overview

An perfect opportunity has arisen for a 6 month fixed term/ secondment for someone to gain experience or enhance their skills in our established team as a band 6 Mental Health Practitioner. We are happy to support a current band 5 practitioner to gain band 6 skills. The post provides a good work life balance, working 9-5, 3 days a week (22.5 hours total). We care for Older People based in the coastal communities of Suffolk.

Our friendly and supportive team provides a community-based service to people of any age with a dementia diagnosis, and to Older People with a functional diagnosis and complex needs.

We are proud to be accredited with the Royal College of Psychiatrists as providing high quality care and good practice. This is important to us, as is our staff wellbeing; we have a Wellbeing Champion in the team and make sure we meet up for Wellbeing Lunches, have away days as well as occasional social nights out.

We are looking for someone who is interested in working in Older People’s services, has an interest in physical as well as mental health and who has an understanding of assessment, the referral process, case handling and working collaboratively with the wider MDT.

Responsibilities
  • Holding a caseload of patients with organic and functional presentations under CPA and NCPA
  • Assessing and managing risk.
  • Using Dialog+ where possible and Recovery Care Planning with service users and carers.
  • Managing your own diary and planning your day.
  • Attending MDT meetings and ward review meetings.
  • Duty working 1 half day per week.
  • Communicate effectively through notes on Lorenzo and letters to GPs and other involved parties.
  • There is an opportunity to learn to take ECG readings and to learn phlebotomy.
  • Driving is essential for this role.
  • Full induction will be given.
