Responsibilities
  • The post holder will work as part of the Learning Disability team based at Denbighshire, ensuring interfaces between the Local Authority and neighbouring services within the Division, including community mental health teams, in-patient services, and other statutory agencies and the voluntary sector.
  • The post holder will ensure they deliver evidence-based, safe, effective interventions and clinical care, following the nursing process.
  • The post holder will follow the service principles and strategy for people with a learning disability while delivering safe, effective and high-quality specialist service in line with agreed standards.
  • The post holder will have excellent communication skills and be a visible and accessible member of the team; patients and their families can access them for assistance, advice and support.
  • The post holder will be expected to communicate and receive highly complex and sensitive information, analyse and interpret it, and act appropriately where there may be barriers to acceptance.
  • The post holder will have a good understanding of the CHC process and be involved in developing pathways and the Maturity Matrix.
  • The ability to speak Welsh is desirable; Welsh and/or English speakers are equally welcome to apply.
