Job description

We are seeking to appoint an enthusiastic Community Hub Coordinator to support our teams and service users within Community Health Services for Nursing and Therapy based primarily at Melton Mowbray Hospital. The role will also require working from our other bases in Rutland Memorial Hospital in Oakham and Syston Health Centre in Syston as and when required. In addition to general administrative duties the role will include using the electronic patient record system (SystmOne), completing electronic referrals, tasks and assisting where appropriate with data collection for audit purposes, data entry, along with supporting Core Community Nursing and Therapy staff, Team Leads and clinicians, ordering and monitoring of equipment and stores.
