Job Purpose: Under the direction of the Team Manager, to provide planned and staged support services to children, young people and their families

• In Interventions where needs are increasing in complexity and families require support to remain together
• In Targeted Services where complexity of needs requires additional measures to sustain appropriate care and prevent young people from being accommodated
• To help maintain children in placements and to support in transitions
