Job Description

As a Community Care Professional,you will provide essential care and support to clients in the community —enabling clients to maintain dignity, independence, and quality of life within their own homes;

Key Responsibilities
  • Deliver high-quality, person-centred care in clients’ homes
  • Support with daily living tasks such as washing, dressing, mobility, light household duties and meal preparation
  • Provide companionship and build positive, trusting relationships
  • Assist with medication prompts and general wellbeing monitoring
  • Accompanying your client to appointments, or a visit to the shops
  • Record and report care activities accurately
  • Promote independence, respect, and choice in every interaction
  • Communicate effectively with clients, families, and team members to ensure seamless care coordination
  • Support clients in maintaining their independence and social engagement within the community
