A community health organization is seeking an enthusiastic registered nurse to join their team in Chelmsford. This role involves managing patients with heart failure, collaborating with healthcare partners to ensure high-quality care and preventing hospital admissions. Candidates must have at least two years of cardiology experience and the ability to work autonomously in various community settings.
An exciting opportunity has arisen for an enthusiastic, motivated and experienced registered nurse to join our friendly, busy, yet rewarding team within the community of Mid Essex.
You will be working within the community to support the management of patients who have heart failure. Working in collaboration with our MSE partners to support a seamless patient pathway and to prevent hospital admissions and readmissions.
This post holder will be responsible for ensuring high quality, evidence-based care for patients with the diagnosis of Heart Failure, their families and carers in the community. Managing patients at home, remotely or in the nurse-led heart failure clinics.
The post holder will:
