We are looking for highly skilled,enthusiastic, dynamic self-motivated leader to lead our Frailty Team based atClarendon, Hebburn and Community Falls Team based at Washington Galleries.

You will be responsible for theleadership and management of the Frailty Team consisting of Band 7 specialistnurses and Community Falls Team consisting of Band 6 nurses, physiotherapistsand HCAs. You will be responsible for the day-to-day Leadership and Managementof the Team and will be responsible for governance and assurance.

Main duties of the job

The Frailty Team normal workinghours work Monday Friday 08.30 - 5

Community Falls Team working hours08.30-4.30

The post holder will beaccountable for the overall management and development of the team andsupervision of staff and ensuring the coordination of services provided,optimising the cost-effective use of all resources.

Line management of the team, withoverall accountability and responsibility for the delivery of all aspects ofpatient / client care through the provision of effective clinical andmanagerial leadership.

To lead, motivate and support theteam approach to delivering new and innovative models of care for patients /clients.

To lead a continuously improvingservice, by ensuring systems are in place to support clinical governance,quality monitoring and ongoing research and audit of practice.

Person Specification
Skills and Knowledge
  • Evidence of ongoing professional development, willingness to maintain and extend skills and knowledge.
  • Excellent inter-personal and communication skills in order to forge relationships with colleagues and others for the benefit of patient care and caseload coordination.
  • Expert assessment, analytical, interpretation and clinical decision making skills.
  • Have complete ENCOP at foundation level or willing to complete.
  • Ability to work under pressure and achieve tight deadlines in a complex / changing environment.
  • Have completed ENCOP at specialist level.
Physical Skills
  • Able to move and handle safely without causing harm to themselves or others.
Qualifications
  • Registered Nurse/AHP.
  • Degree Level Training.
  • Post registration qualification specific to role.
  • Non-Medical Nurse Prescribing.
  • Advanced clinical skills.
  • Masters in health / leadership related subject.
Experience
  • Experience within a community setting as a Nurse Practitioner or Community Matron and caseload holder.
  • Significant experience at Band 6 level or above in a community setting.
  • Multi-disciplinary / multi-agency working.
  • Portfolio of evidence of continuous professional development.
  • Experience of leading clinical development.
  • Experience of managing change.
  • Leadership role.
  • Complex caseload management.
  • Ability to undertake audit.
  • Involvement in service review and development.
  • Experience of working in Frailty and Older Persons.
