Overview

The Intensive Community Support for Dementia (ICSD East) is a hospital avoidance team providing high standard, patient‑centred care for people with dementia in their own home or community settings. Based at Alderney Hospital, the team works with older adults in the East of Dorset, supporting the Older People’s Community Mental Health Teams, preventing unnecessary admissions and facilitating safe planned discharges from organic wards.

Role Summary

We are looking for a full‑time Band 6 Registered Mental Health Nurse to join our dynamic team in a permanent position. You will support older people with dementia at the community level, working closely with carers and multidisciplinary agencies.

Responsibilities
  • Gatekeep the organic beds at Alderney Hospital, coordinating shifts and ensuring all visits are covered.
  • Triage new referrals and ensure patients are assessed within our timeframes.
  • Lead and support junior staff on shift during patient visits.
  • Provide rapid, specialist and intensive short‑term, care‑planned support at home.
  • Work closely with carers and other agencies in older people’s health and social care services.
  • Support the discharge of people with dementia from the organic wards at Alderney Community Hospital into community placements, ensuring effective discharge and transition.
Qualifications
  • Band 6 Registered Mental Health Nurse, full time, permanent.
  • Experienced Registered Nurse with knowledge of dementia in community settings. RNMHs preferred; experienced RGN or RNLD with evidence of community and dementia experience also welcomed.
  • Dynamic, highly motivated nurse with exceptional assessment, risk management and care formulation skills and experience with older adults and dementia health in a multidisciplinary setting.
  • High standard communication skills and clinical decision‑making essential.
  • Senior nursing staff must be able to process information accurately and quickly (virtual ward context).
