Job summary

A leading healthcare provider is seeking an onsite Independent Prescriber – Clinical Pharmacist. This role involves conducting virtual medication reviews, answering clinical queries, and managing complex medication issues. Candidates must have a Pharmacy degree, GPhC registration, and experience supervising junior pharmacists. Excellent communication skills and a proactive approach are essential. This position is part-time, working 22.5 hours over 3 days per week, providing significant contributions to patient care in a supportive environment.

Job Summary

22.5 hours per week over 3 days – Onsite Independent Prescriber – Clinical Pharmacist – Portland Medical Centre, part of Bourne Health.

Key Responsibilities
  • Virtual medication reviews for repeat prescription requests from patients, care homes and pharmacies.
  • Answer clinical medication queries from GPs, practice nurses and other healthcare professionals.
  • Respond to complex medication queries from patients.
  • Liaise with secondary care teams as needed for medication issues.
  • Review hospital discharge letters and perform medication reconciliation; manage additional consultations after prolonged admissions.
  • Handle complex medication queries from community pharmacies when prescription clerks cannot resolve them.
  • Conduct audits for medication safety and recommend process improvements.
  • Process shared‑care medication requests from secondary care specialists.
  • Perform face‑to‑face medication reviews where indicated.
  • Review and act on MHRA medication safety alerts as received.
  • Prescribe repeat medications.
  • Review acute requests and switch over to repeat medications where appropriate.

Internal: Senior Management team, Partnership Board, colleagues within The Bourne Partnership. External: suppliers of services, other GP practices and practice groups, other external organisations.
