Qualifications

• Minimum 12 months experience working in general practice or primary care.
• Ability to travel to the practice site or reside within reasonable commuting distance.

Responsibilities

• Act within professional boundaries supporting a team of pharmacists and GPs.
• Conduct polypharmacy medication reviews and SMRs.
• Provide expert advice on medications to other clinicians and patients.
• Deliver clinical leadership on medicines optimisation and quality improvement.

Skills

Excellent communication and interpersonal skills

Experience using EMIS

Education

Registered Clinical Pharmacist with the General Pharmaceutical Council (GPhC)

Completion of the CPPE Primary Care Pathway or evidence of exemption
