Overview

Clinical Nurse Assessment Specialist (RGN/RMN/RNLD)

Location: This is a nationwide role, covering our existing and new portfolio of homes. The ability to drive and access to a car is essential.

Contract type: Full time, 40 hours per week (Monday to Friday)

Salary: £40,000 per annum, plus £3,000 car allowance

Join us as our new Clinical Nurse Assessment Specialist here at Exemplar Health Care. Reporting to the Senior Clinical Assessment Specialist, you’ll work flexibly as part of a highly motivated Clinical Assessment Team, supporting our specialist care homes to respond to an ever-growing number of service user referrals. We have an ambitious growth plan to expand our portfolio of homes over the coming years, and we are looking to add this role to enhance our established function. You will need to demonstrate passion, commitment and determination to ensure every referral is managed to the highest standard and that our customers receive an exemplary experience.

Role responsibilities
  • Support care homes to complete clinical pre-admission assessments, working closely with operational and clinical colleagues. Travel to different care settings nationwide within the allocated working day.
  • Produce individual cost per care assessments that demonstrate the specialist care package and justify the interventions and costs. This can be completed in the office or remotely from home during working hours.
  • Coach Clinical Nurse Managers and Home Managers in the assessment and costing process and contribute to training delivery.
  • Support processing of referrals, as requested by the Senior Clinical Assessment Specialist, working with the Referrals and Occupancy Management Team.
  • Uphold the company vision and drive the Exemplar Health Care strategy.
  • Where required, support homes with reassessment of existing service users.
  • Build relationships with new and potential referrers.
  • Attend conferences, events and networks to build relationships and raise Exemplar Health Care’s profile as required.

Qualifications

To apply, you must have a first-level nursing registration with a valid Nursing and Midwifery Council (NMC) PIN. We’re looking for Registered Nurses from all backgrounds (Adult, Mental Health and Learning Disability Nurses).
