At the Royal Devon University Healthcare NHS Foundation Trust we are committed to delivering compassionate, patient-centred care that keeps people safe and well in their own homes. Our Urgent Community Response (UCR) Services are at the heart of this, helping prevent unnecessary hospital admissions, improving patient outcomes and supporting timely discharges.

We are seeking a Clinical Team Lead with drive, clinical expertise, and leadership skills to empower their team and shape the future of Urgent Community Response across two localities, covering Honiton, Ottery & Cranbrook (HOSM) and Seaton, Axminster & Sidmouth (SAS) clusters.

Main duties of the job

As a key member of the Royal Devon University Healthcare NHS Foundation Trust, you will be joining an exceptional team working together to help our patients stay healthy, when they need it most. For more detail about the main duties of this role, please see the job description section below.

About us

Stretching across Northern, Eastern and Mid Devon, we have a workforce of over 15,000 staff. Our core services cover more than 2,000 square miles across Devon, and some specialist services extend to Cornwall and the Isles of Scilly. We provide emergency, specialist and general medical services through North Devon District Hospital and the Royal Devon and Exeter Hospital. We also offer integrated health and social care services in community inpatient hospitals, outpatient clinics, and in people’s own homes, including primary care, specialist community services, and Sexual Assault Referral Centres (SARC).

Our state-of-the-art equipment and links to local universities support research and innovations to provide world-class care. It’s an exciting time to join as we continue our integration journey.

Job responsibilities

More detail about the role is available in the Job Description. The Role:

  • Lead with impact across the teams, providing clinical governance, guidance, and support.
  • Manage a complex caseload, delivering advanced assessment, treatment, and advice to patients and carers, and share skills with the team.
  • Mentor, supervise, and inspire your team, fostering professional growth and a culture of excellence.
  • Collaborate across services (GPs, hospital staff, community services, Social Care, and voluntary organisations) to ensure seamless, high-quality care pathways.
  • Drive service development and improvement to enhance patient outcomes and team performance.

  • Is confident in service development, governance, and clinical decision-making.
  • Is proactive, organised, and committed to achieving excellence in patient care.
  • Is able to work well under pressure and collaborate to achieve the best outcomes.
